Overview
So far in this course, you have iden�fied a health or healthcare problem and completed your needs assessment for this program. Also, you explored various program planning models and
compared them to evaluate their strengths and limita�ons. Now, you are ready to design your program model.
Based on the findings from your program planning models’ analysis, you decided to use the logic model in designing your program. You will complete the logic model diagram over a few
weeks to support your final presenta�on to your senior management. This will help them visualize your plan well before the program implementa�on. This assignment will help you and
senior management know the resources you need to implement this program, the ac�vi�es that will help you achieve the program goals, and the outcomes you want to set for the program.
This is one of the essen�al steps before program implementa�on, and it helps to iden�fy relevant implementa�on challenges.

Write a brief reflec�on on three logic model components, explaining your reasoning behind your decisions for each.
Specifically, you must address the following rubric criteria:
